We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.
If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.
Not all providers participate in every listed insurance plan. Please confirm with your insurance company before scheduling.
You are responsible for any charges not covered by insurance, including copayments, coinsurance, and deductibles, which are due at the time of your appointment.
Referrals may be required. HMO plans always require one. If you don’t obtain a referral and your insurance denies coverage, you will be responsible for the full cost.
Please note that at this time, we do not accept Medicaid.
If a procedure or medication requires prior authorization, we will assist in obtaining it from your insurer.
